As another benefit of Prima membership, we maintain a library of forms, policies, and other materials that may be downloaded and modified for use in your office. These forms are provided by Dr. Neil Gailmard and other Prima members who are happy to share this information.

Clinical Forms – offers a variety of forms useful in the day-to-day management of the practice. Among the samples available are patient history forms, exam and re-test forms, a superbill, and the use of retinal screening.

Staff Management Materials – includes many useful tools to provide assistance on the human resources side of management. Samples include employment agreements for associate ODs, job descriptions, employment ads, staff evaluation forms, and employee handbooks.
